Reserved Books

      A fine of P5.00 an hour and every fraction thereof, or a maximum of P50.00 a day, is charged for a reserved book not returned on time.

First offense merits cancellation of overnight loan privilege of the student for one (1)week.

Second offense merits cancellation of borrowing privileges of all books for one (1)week.

Third offense merits cancellation of library privileges. Violators will not be allowed to enter the library for one(1) week.

Fourth offense merits cancellation of library privileges. Violators will not be allowed to enter the library for two(2) weeks.

Penalties will be noted on the Athena Circulation Module for each library clientele who failed to return library materials promptly on due dates and after receipts of Recall Slips.

Library privileges of students who have been delinquent or have been found violating library policies on three(3) consecutive occasions in a given semester shall be suspended for one(1) month.

Lost Books

      Lost books must be reported at once, and be replaced within two(2) weeks from the date it was reported at the Circulation Section. Failure to replace or pay for the book within the allotted time means suspension of borrowing privileges.

Users will be required to pay for the actual cost of the replacement, handling, and other processing charges.

Clients must pay the processing fee of P100.00 to cover expenses for barcode sticker, spine label, plastic cover, book card, book pocket and date due slip.

Clients will pay the corresponding fine from date due to dale loss was reported.

Lost book must be replaced with exactly the same copy or newer edition if available. In case the same book is no longer available, ask the University Librarian what book may be substituted for the book.

Payment for lost book is done at the Finance Office after issuance of payment slip from the Circulation Section.

The official receipt is required to be returned to the Circulation Section after payment together with the payment slip for recording in the Book Fine Record and clearance.

Damages

      All patrons are responsible for the materials borrowed from the University Library until these materials are returned.

They are also responsible for any changes incurred for damages library materials.

Library materials damaged beyond normal use must be replaced with the exact copy of the material.

Damaged book must be replaced with exactly the same copy or newer edition if available.

In case the same book is no longer available, the borrower must ask the University Librarian what book may be substituted for the damaged book.

Finding missing pages or any form of damage in any just borrowed library material must be reported immediately to the Circulation Section.

Theft, Vandalism & Mutilation

      Books and other library materials are to be used properly: tearing, high-lighting and writing on them, or using them as writing boards are to be avoided. Mutilation of any library material is strictly prohibited.

Users caught bringing out library materials taken without permission or proper check out procedure shall be directed back to the Circulation Counter to complete the check-out process.

Offenders will be investigated and will undergo due process. They will:
(a) be required to pay for the actual cost of repair and other processing charges
(b) make a written apology, and
(c) will be reported to the Office of Student Affairs for appropriate disciplinary action.
